Unite California is a coordinated care network of health and social care providers.

Partners in the network are connected through a shared technology platform, Unite Us, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite California is sponsored by the Community Connections Partnership, led by Kaiser PermanenteCommonSpirit Health, and Blue Shield of California. Additional network sponsors include Adventist Health Lodi Memorial.

The Unite Us network makes it possible for SAR navigators to support adults and youth in the Sacramento community who have been or might be involved in the justice system. The network makes it possible for us to coordinate and connect those we serve with the resources they need to be successful and heal from the trauma they have experienced.

-Daniel Silva, Self Awareness & Recovery (SAR), a non-profit that started in the CA prisons, and has since expanded to the greater Sacramento area. They focus on Trauma Informed Healing and work to help those re-entering our community as well as dedicating time to the youth of our community in an effort to prevent future incarcerations.
