About Us

Unite California is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite California is sponsored by the Community Connections Partnership, led by Kaiser Permanente, CommonSpirit Health, and Blue Shield of California. Additional network sponsors include Adventist Health Lodi Memorial, the City of Long Beach, and Santa Cruz HIO. The network is supported by a California-based Unite Us team focused on community engagement, network health and optimization, and customer success. Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers. In addition, county agencies, city governments, and public health care systems receive 25 web-based licenses at no cost.

The Community Connections Partnership

The Community Connections Partnership was launched in 2020 by three California health systems — Blue Shield of California, CommonSpirit Health, and Kaiser Permanente  — in collaboration with dozens of community-based partners and technology company Unite Us. Together, the Partnership is committed to making a collective impact on care quality and health equity for California’s most vulnerable populations by finding new ways to address the social factors that contribute to negative health outcomes, from inadequate living conditions and poor nutrition to social isolation.

Community Roots

The Unite Us network makes it possible for SAR navigators to support adults and youth in the Sacramento community who have been or might be involved in the justice system. Unite California is currently live in Napa, Solano, Sacramento, Yolo, Placer, Mariposa, Merced, San Joaquin, Stanislaus, Los Angeles, Kern, El Dorado, Tulare, Kings, Fresno, Santa Cruz, Butte, Marin, Sonoma, Madera, Orange County, Ventura, Riverside, San Bernardino, Alameda, Santa Clara and Contra Costa counties.

Our community champions include Kern Community Foundation and Community Health Councils. Our convener is HandsOn Central California.

The Unite Us network makes it possible for SAR navigators to support adults and youth in the Sacramento community who have been or might be involved in the justice system. The network makes it possible for us to coordinate and connect those we serve with the resources they need to be successful and heal from the trauma they have experienced.
-Daniel Silva, Self Awareness & Recovery (SAR), a non-profit that started in the CA prisons, and has since expanded to the greater Sacramento area. They focus on Trauma Informed Healing and work to help those re-entering our community as well as dedicating time to the youth of our community in an effort to prevent future incarcerations.

This platform is literally going to unite us. Even as someone who has been a client before and needed services, this puts me at ease to know I won’t be jumping through hoops to get connected to services. It’s going to unite and bring together all of the key players, I can’t wait.

Unite Us has been very beneficial for Garden Pathways in our day-to-day operations. We are able to effectively manage our referral system and monitor duplication of services. Our staff was blown away by the quick responses to their referrals - they didn’t have to do any of the typical back and forth to get updates on the referral. In one case in particular, a partner couldn’t accept a referral for various reasons. But, instead of simply rejecting it, they sent the referral on to another organization that was able to help. This was so helpful, warm and personal!

Founded in 1964 by legendary civil rights leader, Cesar Chavez, the Cesar Chavez Foundation is a nonprofit that inspires and transforms communities by providing critical services that address essential human, cultural and community needs of Latinos and working families. Through its partnership with Unite Us, the Chavez Foundation is helping senior residents and working families of its affordable housing communities connect with local resources.

Unite Us has increased our referrals and outreach efforts, diversified our incoming referrals, and helped us launch a new program. Not only has it bolstered our exposure in the community, increased efficiencies, and improved the continuum of care, it has led to larger grant awards from funders. Client-based referral platforms are the wave of the future; if you are not a part of the wave, you and your nonprofit may be left behind. Blue Shield of California’s partnership with Unite Us empowers us to help provide connections to resources for the people in our communities -- and allows us to track these referrals to determine whether they are meeting people’s needs. Over the last decade, many veterans have been lost in referrals, making it hard for them to get the services they need. Unite Us helps the Los Angeles County Veteran Peer Access Network keep veterans at the forefront, getting them connected to everything they need, leaving nobody left behind.

Unite Us has helped connect a disconnected society. Challenges are everywhere: lack of trust in the network, peers' ability, and service follow-up. Unite Us connects the service availability to trained peers in the Los Angeles County Veteran Peer Access Network, who can conduct follow-ups with ease. Almost 60 veteran organization partners in LA County means that together we are accomplishing more, decreasing veteran issues that were once difficult to overcome. Unite Us is helping VPAN achieve our mission by bringing a coordinated network that provides us not only a platform but a new method of access to care for our veterans and families in LA County.

Unite Us is an integral partner in our Vaccine+ work in Los Angeles, enabling us to utilize vaccination appointments to connect the communities hardest hit by the pandemic with the resources they need to improve their health and well-being. Thank you, Unite Us, for connecting Angelinos with the incredible organizations that serve this community and helping us achieve our broader mission of advancing health equity! Within 2 weeks of launching this initiative, we have surpassed 250 assistance requests as our team begins to initiate referrals within the Unite Us CA network!

At Kaiser Permanente, we look at the big picture when it comes to health. Unite NYC

Unite NYC is a coordinated care network of health and social care providers that is launching on April 14th and sponsored by funding partners currently led by Public Health Solutions and AIRnyc. In 2018, our work with Public Health Solutions began as a small food security grant-based pilot (FNS Bundle) in the Bronx. The current city-wide expansion is part of a rapid response to COVID-19.
